Partnership Integration Coordinator (Ref: 25346)



The post holder will serve as a service ‘Coordinator’ at the local operational level, maximising on front-line delivery. The post holder will work to support operational delivery of the Women’s Pathfinder to ensure delivery is evidence based, based on best practice and interacts effectively with a wide range of other services. This will require the post holder to lead on local communication and engagement activity in relation to a Whole System Approach, particularly with partners such as third sector, local authorities and sentencers, including Her Majesty’s Court Service (HMCTS) and Crown Prosecution Service (CPS) locally.

Partnership Support Responsibilities
Co-ordinate multi-agency activity within local geographical areas, on an organisational basis Responsible for working with agencies to establish single entry point for women coming into contact with the Criminal Justice System.
Establish secure enhanced referral routes for priority group into local services.
Enable effective front-line operations by providing an ‘active problem-solving’ approach for some of the most challenging and complex cases. Support engagement activity between prisons and local provision to ensure seamless transition arrangements. Responsible for maintaining accurate information on relevant approved databases.
Share and promote best practice and support the identification of areas for development and service improvement across services and partner organisations.
Ensure the effective administration of referrals from across a range of agencies in line with guidance.
Prepare, maintain and collate case and other records, files and management information, in accordance with the standards required.
Coordination and organisation of multi agency meetings, including booking rooms, contacting attendees and preparing materials. South Wales and Gwent only – work with local Women’s Pathfinder delivery providers as part of the Whole System Approach Service Delivery Model, commissioned by Gwent Police and Crime Commissioner in partnership with South Wales Police and Crime Commissioner, HMPPS and Welsh Government.

Use communication effectively
Receive and deliver gender informed training to staff across HMPPS who work with women, including CRCs and other external agencies.
Receive and distribute information and communications in an appropriate manner, e.g. telephone, paper, email. Coordination and organisation of multi agency meetings for the partnership, including booking rooms, contacting attendees and preparing materials. Deal and communicate fairly, timely and effectively with agencies and partners.
Act as a point of contact for the programme of work as required.

Overview of the job

The job holder will undertake the full range of work with offenders before and after sentence. This will include assessment, sentence implementation, offender management and producing reports. The job holder will provide case management support to a full range of offenders utilising service procedures and practice directions that underpin professional judgement.

Summary

To assess and manage the risk (including risk management plans and escalation) posed by offenders to protect victims of crime and the general public by:

Liaising, providing information and advice to criminal courts, criminal justice agencies and other partner agencies
Supervise and manage risk of those offenders subject to community sentences, during and after custodial sentences.
Work with other agencies and groups to prevent crime and meet the needs of victims and offenders.

Responsibilities,Activities & Duties

Probation Services Officers may be required to undertake any combination, or all, of the duties and responsibilities set out below.

To undertake the full range of offender management tasks with offenders assessed as low or medium risk of harm and to support the Probation Officer grade in high risk cases.
When providing case manager support, to contribute to the delivery of the Risk Management plan and report significant changes relating to risk of harm and/or of reoffending or any non-compliance within enforcement procedures
To use computer based systems to produce, update and maintain records and other documentation within agreed timescales
Ensure effective referrals to services and facilities and communicate with offender management staff, interventions staff, service providers and external agencies to review progress and associated risks.
To undertake prison, home or alternate location visits as required in accordance with service procedures and policies.
To undertake work in the court setting, including the completion of appropriate reports on cases and prosecution of breaches.
To provide cover within teams as required
To deliver and co-lead accredited programmes commensurate with grade
To conduct mandatory alcohol and drug tests as required, and to follow prescribed medication procedures
Carry out safeguarding children duties in accordance with the NPS statutory responsibilities and agency policies
Demonstrate pro-social modelling skills by consistently reinforcing prosocial behaviour and attitudes and challenging anti-social behaviour and attitudes.
